Healthcare quality uplift · System optimisation
Muhammad Azhar Hussain — MLT, M.Sc, M.Phil
Quality that shows up in the numbers.
Lead assessor and auditor for ISO 9001 and ISO 15189 — and twenty years running the laboratories, networks and P&L those standards describe. I help diagnostic services and hospitals close the distance between the standard on paper and the practice on the floor, then leave behind a system their own people can run.
Erbil, Kurdistan Region of Iraq — open to selective advisory engagements
Certifications and accreditations are cumulative across successive employers. Growth and renewal figures were achieved at Metropole Laboratories, detailed under Record & evidence.
01 · The brief
The situations I am usually called into.
Every engagement starts from a real operational pressure, not a generic maturity model. If one of these sounds like your facility, the conversation will be short and useful.
The assessment date is fixed and the gap is wider than the calendar.
ISO 15189, CAP or JCI is coming, the documentation is partial, the evidence trail is thin, and nobody can say with confidence what will be found. You need a realistic gap position and a sequence that actually fits the remaining weeks.
Compliance has quietly become a second job.
Procedures were written around the standard rather than around the work, so staff maintain the system separately from doing their jobs. It holds while someone pushes it, and decays the moment attention moves elsewhere.
Turnaround time is slipping and nobody can point to where.
Clinicians complain, the laboratory defends, and neither side has instrumented the specimen lifecycle. Without a measured view of collection, transport, processing, verification and release, every explanation is anecdote.
The software was bought before the workflow was designed.
An LIS or QMS implemented as an IT project rather than a clinical operations project. Now it is a second system to feed, staff work around it, and the data it produces cannot be used as evidence.
The common failure is not knowledge of the standard. It is the translation between the standard, the workflow and the software.
That translation is the work I do.02 · Services
Four ways I work with healthcare organisations.
Each line can run as a standalone project or combine into a single programme. Scope, deliverables and the point at which you take ownership are agreed in writing before work begins.
Accreditation & compliance readiness
From an honest gap position to a successful assessment — and an evidence system that holds between assessment cycles rather than being rebuilt before each one.
Gap assessment and risk register · document architecture and controlled SOP set · internal audit programme and auditor training · CAPA system · IQC and PT/EQA programme design across CAP, EQAS, UK NEQAS and RIQAS · mock assessment · assessment-day support.
Laboratory commissioning & service design
Standing up a new laboratory, STAT unit, blood bank or collection network — designed once, correctly, instead of corrected for years afterwards.
Layout and workflow design · analyzer and assay selection with test-menu strategy · method verification and validation protocols · biosafety and biorisk design · staffing and competency model · commissioning plan through to go-live.
Operational excellence & throughput
Making the service measurably faster, steadier and cheaper to run — using the same evidence base that satisfies the assessor.
End-to-end process mapping · bottleneck and root-cause analysis · KPI framework with agreed definitions · management dashboards · workforce competency programme · service-level design for clinical and B2B clients.
Digital systems & quality informatics
Specifying, selecting and implementing laboratory and quality software from the clinical workflow outward — so the system produces evidence rather than extra work.
Domain-led requirement specification · vendor evaluation and selection support · workflow and configuration design · interface and integration specification · validation protocols and records · adoption, training and post-go-live stabilisation.
03 · Approach
Measure the operation before judging the documents.
Consultancy that ends when the consultant leaves is a cost, not an investment. Every engagement is structured so that ownership transfers to your team by the final phase.
Baseline
Two to five days, on site or remote. The operation is measured — volumes, turnaround, rework, staffing, bottlenecks — and read against the target standard. You receive a findings report and a prioritised risk register, useful even if the engagement stops here.
Redesign
The workflow is fixed first, the documentation second. Document set, audit calendar, KPI definitions, competency matrix and system changes are written for your facility, your test menu and your staffing reality — with owners, sequence and dates.
Embed
Built into the daily work alongside your team, not instead of it: document build, training, internal audit cycles, CAPA discipline, method verification, PT/EQA programmes, mock assessment and corrective sprints.
Prove
The baseline is re-measured and the movement is shown. Trained internal auditors, a running management review cycle and live dashboards transfer to your team, so the system survives staff turnover and the next assessment.
Advisory retainer
A defined monthly commitment, remote-first with scheduled site visits. Suited to organisations that need continuity of expert judgement rather than a one-off project.
Project engagement
Fixed scope to a defined milestone — accreditation achieved, laboratory commissioned, system live. Priced against the deliverable, with the success criteria written down at the start.
Interim quality leadership
Embedded part-time quality leadership while you build or recruit the permanent function, including mentoring the person who will eventually hold it.
04 · Record & evidence
Twenty years, and what moved.
The career in four chapters, most recent first — each described as situation, intervention and result. Figures are those achieved within the named organisation; commercially sensitive detail is deliberately withheld.
Laboratory depth extended across a whole hospital
The English Hospital · Erbil
Quality Manager — Laboratory Technical & Quality Leadership
A hospital requiring laboratory accreditation and organisation-wide quality development simultaneously, across clinical and support services with differing maturity and no shared evidence framework.
Laboratory-grade quality discipline is being extended outward into the wider hospital architecture, translating multiple standards into one coherent set of workflows rather than parallel compliance programmes — with digital systems supplying the evidence layer.
- ISO 15189:2022 and CAP LAP implementation under way alongside organisation-wide quality-system development
- JCI 8th Edition and ISO 7101:2023 requirements translated into practical, auditable workflows
- Domain leadership for LIS, IQMS and LMS digital-transformation programmes
Accreditation and growth as one operating system

Manager — Quality & Business Growth
A multispeciality reference laboratory processing more than 25,000 tests per month, plus a new integrated clinical-services platform being stood up alongside it — with international accreditation and regional market growth competing for the same management attention.
As a founding-team and core-management member, operations, quality and commercial development were run as a single system rather than three departments. The quality architecture was built to serve turnaround and reliability, and that reliability then became the commercial proposition to institutional clients.
- ISO 9001:2015 certification (URS) and ISO 15189:2022 accreditation (PNAC) achieved
- Zero major non-conformities across the assessment cycles
- CAP, EQAS, UK NEQAS and RIQAS proficiency-testing programmes established and managed
- 50+ new B2B accounts secured across public and private sector partnerships
- >80% client renewal sustained and +25% regional test-volume growth within one year
- Design, quality-infrastructure and laboratory-development input to GPH as a core foundation-team member

Laboratory Manager to General Manager
A growing diagnostic business spanning a central laboratory, blood bank, STAT services and collection points, where each site had drifted into its own practice — making quality, turnaround and cost impossible to compare or control across the network.
A single ISO-aligned laboratory quality management system was designed to work identically at every site, supported by in-house LIS automation and analyzer interfacing so that performance became measurable rather than reported.
- ISO 9001:2015 certification and PHC licensing achieved for the network
- A PBTA-licensed blood bank established for a mother-and-child hospital
- Served on the Management Board of Mother & Child Hospital — a key client — supporting MSDS compliance, ISO 9001, infection control, patient safety and PT/EQA management
- Functional leadership of LIS automation, analyzer interfacing and KPI dashboards
- Network expanded across central, STAT, blood-bank and collection services under one operating standard

Laboratory Technologist to Assistant Manager
A 500-bed tertiary-care teaching hospital with inpatient occupancy reported above 80%, carrying the full weight of national regulatory compliance across every clinical and managerial service.
Laboratory operations were run from the bench upward while participating in core hospital management — implementing the Punjab Healthcare Commission Minimum Service Delivery Standards, the mandatory facility-wide framework, alongside ISO 9001 quality systems.
- PHC Minimum Service Delivery Standards implemented facility-wide alongside ISO 9001 quality systems
- Infection control and patient-safety programmes supported across the hospital
- QA/QC and PT/EQA management established across core laboratory disciplines

05 · The practice
Consulting that produces software, and software that carries the consulting.
QatalystCore is a joint venture with Iftkhar Ahmed, providing consulting and building tools, software and ERP for healthcare, diagnostic and medical centres — with QMS handling and compliance assurance embedded in the product rather than bolted on afterwards. Every module began as a problem that could not be solved with a spreadsheet during a live implementation, which is why the workflows match how laboratories actually run.
How QatalystCore works
Founded and run by two practitioners who between them assess to the standards and build the systems that satisfy them.Who it serves
OperatingHospitals, diagnostic laboratories, medical centres and multi-site healthcare groups across Iraq, Pakistan and the wider region.
Compliance built in, not bolted on
Core principleQMS handling sits inside the product. Evidence for ISO 15189, ISO 9001, CAP and JCI is produced by the daily workflow rather than assembled before an assessment.
Consulting that informs the build
AvailableThe four disciplines above are delivered through the venture as well as personally — and every implementation feeds the roadmap. Nothing is specified from a requirements document alone.
Accreditation readiness
AvailableThe most common way an engagement starts: a readiness programme run to a fixed date, with the platform standing up beside it so the client owns a working system once the assessor has gone.
Training & internal auditor development
AvailableStandard interpretation, internal audit skills, biosafety and biorisk, competency assessment and CPD design — delivered standalone or alongside a platform rollout.
The QatalystCore platform
Maturity is stated plainly. Advanced builds and prototypes are not presented as commissioned enterprise systems.IQMS — Integrated Quality Management System
Active developmentOne controlled platform carrying the whole quality system, with KMS, LMS and QRS beneath it. The architecture is set out below.
LIS — Laboratory Information System
Active developmentBuilt around the patient–order–specimen–result–report lifecycle: order flow, specimen tracking, analyzer worklists, verification and release, billing and report delivery.
Controlled document packs
DeliveredDiscipline-specific QMS architecture written to the standard — policies, SOPs, forms and records, version-controlled and ready to load into KMS rather than drafted from a blank page.
Courseware & competency packs
DeliveredCourse packages, assessments and competency frameworks built for laboratory disciplines, ready to run in the LMS and to evidence training against the standard.
Healthcare ERP
DirectionA governed ecosystem connecting diagnostic, clinical, quality, workforce, supply-chain, financial and learning operations. Stated as direction, not as a shipped product.
IQMS is the umbrella. Everything reports into it.
Bought separately, a quality system, a learning platform and a document library become three things to reconcile. Here they are one, reading from the systems where the clinical work is already recorded.
01 Systems of record
02 The umbrella
03 What it produces
Compliance assurance — a live evidence position against every standard the organisation is held to, readable on any day of the year rather than reconstructed for a date in the calendar.
Clients can engage the consulting alone, adopt the platform alone, or run an implementation where the two are delivered together — designed as one thing, by the people who will be assessed against it.

Practitioner training, and a research habit that never stopped.
Formal training created a continuous line from diagnostic practice to research, quality systems and evidence-informed decision-making — which is why the advice is testable rather than stylistic.

Personal & social contribution — not a commercial venture
Kaamyab Zindagi — a study community for a purposeful life.
The professional work improves systems. This is the part that concerns the person inside them. Kaamyab Zindagi is a study and reflection community that places scientific literacy and technology alongside divine knowledge — so that faith, evidence and everyday living inform one another rather than compete.
Study & reflection
Structured reading and discussion of divine knowledge, read together with what science and technology genuinely establish — approached honestly, without forcing either side to serve the other.
Personal development
Turning reflection into practice: character, discipline, learning habits, family life and work conducted with integrity — the ordinary decisions where a purposeful life is actually built.
Community & service
A space where people learn together and give something back, and where credible knowledge is made accessible in the language and culture of the people it is meant to serve.
Health · Nutrition · Fitness advocacy
A community advocacy effort translating credible health evidence into culturally relevant prevention, nutrition, movement and wellbeing practice — the same commitment to evidence, directed at families rather than institutions.
Free to participate. Kaamyab Zindagi is kept deliberately independent of my professional practice and of QatalystCore. It carries no commercial relationship with any client engagement.
